Public institutions in Peru, Colombia, and Ecuador often require the submission of birth certificates and other civil status documents issued no more than three (3) months prior to the date of submission. In this situation, you may request a new certificate (duplicate) during consular hours at the Embassy of Ukraine in Peru.
It is possible to obtain duplicates of the following documents:
birth certificate
death certificate
marriage certificate
divorce certificate
name change certificate
Consular fee for issuing a duplicate: 50 USD
LIST OF REQUIRED DOCUMENTS:
1. Valid passport.
If the applicant submits a foreign passport, it is necessary to provide a translation of the passport into Ukrainian. This translation can be done at the Embassy of Ukraine on the day of the appointment.
2. Copy of the requested certificate (if available).
3. For Ukrainian citizens, men aged 18 to 60: an up-to-date military registration document in electronic format (PDF), generated in the Reserv+ application no more than three days before the request to the consular office.

After your documents have been reviewed via email, it will be necessary to schedule a consular appointment and submit the documents in person.

A duplicate certificate is issued in the Ukrainian language. At the same time as obtaining the duplicate certificate at the Embassy of Ukraine, you may also request its translation into Spanish or English. Information on consular translation services is available at the following link.
IMPORTANT: If a new certificate (duplicate) issued at the Embassy of Ukraine in Peru is intended for submission to local authorities in Peru, Colombia, or other countries, it is necessary to complete the procedure of legalization of the consular signature (legalizar la firma consular) at the Ministry of Foreign Affairs of Peru or Colombia. Legalization of the consular signature serves as a substitute for an apostille on documents issued by the Embassy. When requesting a duplicate, please indicate to which authorities the document will be submitted so that we can advise whether legalization of the consular signature is required. If the duplicate will be used in Ukraine, no additional certification is required.
