Certificate of unmarried status

If it is necessary to confirm marital status for the purpose of marriage registration in another country, citizens of Ukraine and persons without citizenship residing in Ukraine and citizens of Ukraine residing abroad shall submit a declaration, the authenticity of the signature on which must be notarised, including by a diplomatic mission or consular office of Ukraine, in which the person declares that he/she has never been married, or that he/she was previously married but now his/her marriage has been terminated, and duly legalised, unless otherwise provided by international treaties of Ukraine, ratified by the Verkhovna Rada of Ukraine.

In the case of Peru and Colombia, Ukrainian citizens may submit a notarised declaration (declaración jurada) as a document of unmarried status (certificado de soltería).

You can apply for this declaración jurada during a consular appointment at the Embassy of Ukraine in Lima or during consular services on site (itinerant consulate) in Colombia.

LIST OF REQUIRED DOCUMENTS

1. Completed and signed application form;

Application form: Download 

2. Valid passport. Original and 1 copy.

3. For Ukrainian male citizens between 18 and 60 years old: an updated military registration document in PDF format, generated no earlier than 3 days prior to application to the consular office via the Reserv+ application.

4. Documents confirming the termination of a previous marriage for persons who were previously married and whose marriage was terminated for any reason.


Consular fee for the signature certification: 20 USD

The consular fee may be paid using one of the following methods:

  1. By bank card using the payment terminal located at the Embassy premises (applicable bank fees may apply).

  2. Through the banking application (available only to persons who have an account with the Peruvian bank BBVA).

  3. In cash, in U.S. dollars, at a bank branch, upon presentation of the bank payment slip issued during the consular appointment.
